Complete and submit both the ADA Internship Application form, and the Central Valley WIC Dietetic Internship Application form - both should be typed.
Request and submit 3 letters of recommendation using the standard ADA Waiver and Reference letter form, 1 from a professor or academic instructor, 1 from a work or volunteer supervisor, 1 from a person of your choice. Letters must be in a sealed envelope, with the signature of the sender (person recommending you) over the back evelope flap. Send all 3 letters together (in the same packet) with your other application materials.
Request and submit official transcripts from all colleges or universities attended.
If you HAVE GRADUATED from college/university, submit a Didactic Program in Dietetics (DPD) Verification Statement.
If you HAVE NOT GRADUATED from college/university, submit a Declaration of Intent to complete your degree before the internship starts. (See your DPD Director for more information.)
Submit forms to participate in D&D Digital Systems computer matching for Dietetic Internships. (If you are currently working for WIC, these forms may be waived. Contact the Dietetic Internship Director for more information.)
Submit a copy of TOEFL results if native language is not English, and you did not graduate from a college or university in the United States. (contact DI Director with questions/concerns.)
Submit a non-refundable $25 application fee made payable to "United Health Centers," money order or cashier's check accepted. Please do not send a personal check.
